Consent to Use Tax Return Information

    Standalone Consent Disclosure Under Treasury Regulation §301.7216-3

    Federal law requires this consent form be provided to you.

    Unless authorized by law, we cannot use your tax return information for purposes other than the preparation and filing of your tax return without your consent. If you consent to the use of your tax return information, federal law may not protect your tax return information from further use or distribution.

    Identity of the document preparation service: Laurasia LLC d/b/a Simple5472 ("Simple5472"), 1309 Coffeen Avenue STE 1200, Sheridan, WY 82801. (Treasury Regulation §301.7216-3 requires this field heading. Simple5472 is a document preparation service — not a law firm, CPA firm, enrolled agent, or tax return preparer as defined in IRC §7701(a)(36).)

    Tax return information covered: The information you enter into the Service for the purpose of generating IRS Form 5472, the accompanying Pro Forma Form 1120, and any Reasonable Cause Statement — including but not limited to your name, address, EIN, LLC formation details, foreign-owner information, reportable transaction amounts, and related supporting facts.

    Purpose of use: Simple5472 uses the tax return information identified above solely for the following purposes:

    • Generating and assembling your requested tax filing documents (Form 5472, Pro Forma Form 1120, and any Reasonable Cause Statement);
    • Delivering those documents to you by email and/or making them available for download;
    • Transmitting those documents to the IRS via fax, if you purchase and authorize the optional fax transmission service;
    • Communicating with you about the status of your filing;
    • Operating, maintaining, and improving the Service.

    Third-party recipients: Your tax return information may be disclosed to the following categories of service providers solely as necessary to deliver the Service:

    • Supabase, Inc. — database hosting, authentication, and file storage
    • Sinch (via Phaxio) — fax transmission to the IRS (only if you purchase and authorize the fax service)
    • Resend, Inc. — transactional email delivery (your filing confirmation email and attached signed PDF are delivered through Resend's email infrastructure)
    • Stripe, Inc. — payment processing (Stripe receives only the data necessary to process your payment; Stripe does not receive your full tax return information)

    Simple5472 does not sell, rent, trade, or otherwise disclose your tax return information for marketing, advertising, profiling, or any purpose unrelated to the delivery of the Service.

    Your consent is voluntary. You are not required to provide this consent. However, if you do not consent, we will be unable to provide the Service because we cannot generate, store, or transmit your tax documents without processing your tax return information.

    Duration: Your consent remains in effect until the earlier of (a) completion of the services requested, (b) your written revocation of consent sent to privacy@simple5472.com, or (c) one (1) year from the date consent is given. Revocation halts future use and disclosure of your tax return information but does not unwind processing already completed (for example, a filing already generated or transmitted to the IRS).

    Retention vs. use after expiration. After this consent expires, Simple5472 retains your filing records under the retention schedule in our Privacy Policy as a business and audit- trail record only. We will not use or disclose your tax return information for any new purpose without renewed written consent.

    Penalties for unauthorized disclosure: Under IRC §7216, any person who is engaged in the business of preparing or providing services in connection with tax return preparation and who knowingly or recklessly discloses tax return information in violation of this section is subject to criminal penalties of up to $1,000 in fines and/or up to one year of imprisonment for each violation. Simple5472 takes these obligations seriously.

    Your consent to the use and disclosure of your tax return information as described above is obtained through the separate §7216 consent checkbox on the checkout page.     This consent document is provided separately from and in addition to the Privacy Policy and Terms of Service. In the event of any conflict between this consent disclosure and the Privacy Policy or Terms of Service regarding the use or disclosure of tax return information, this consent disclosure controls.
